Each coach/volunteer must submit ONE of the following forms

• Adult Coach/Volunteer Registration Form (PDF)
  For coaches/volunteers who supervise club practices/activities where ALL participants are 18 years or older.

• Youth Coach/Volunteer Application (PDF)
  For ALL coaches/volunteers who have direct contact with youth 17 & under during club practices/activities.
  Please do NOT fill in the background screening portion on the bottom of the form.
  
  USATF-NJ has mplemented a required online Background Screening for all Coaches and Volunteers in clubs with members 18 years or younger. Click here for more information about the screening process (recommended reading!). Then, click here to begin the background screening process. All questions about the NJ Background Screening program must be directed to Jane, our screening administrator, at janehp3@msn.com. Please do not contact the USATF-NJ office for details or help on the background screening process as we do not have access to any of the information.
